A NEW
LINE OF DUTY DEATH
BENEFIT FOR OUR MEMBERS

The officers of the Ohio
State Firefighters Association are pleased to announce that,
effective January 1, 2008, a line of duty death benefit
will be available to our members as part of your membership
to the Association. To participate in this program,
departments must pre-register with the Secretary/Treasurer
and agree to keep current beneficiary information for their
members on file.
The Association will pay
a $1000 benefit to a member that dies in the line of duty.
This benefit will be regulated by our Awards Committee and
will be governed by the following rules:

Firefighter or EMT must be a member
of a Fire Department or EMS Squad that is a member of
the OSFA.

The person’s death must occur during or
as a result of an emergency incident. Emergency
is defined as a response for fire, rescue, emergency medical
activity, or the presumption that an emergency has occurred.

In the event of a heart attack, it must
have occurred within twenty-four (24) hours of the emergency.

Preferably the death should be reported
to the O.S.F.A. within twenty-four (24) hours of the occurrence.
The O.S.F.A. will make every effort to deliver this money
within twenty-four (24) hours of the notification.

The member fire department must have pre-registered
with the O.S.F.A Secretary/Treasurer as to the number
of members on their active roster and current beneficiary
information must be kept on file with the department.

The decision of the Awards Committee will
be final.
Current beneficiary information
must be kept on file at the member’s department. It
will also be necessary for the department to provide written
verification that the individual was part of the member
organization. The member department must also provide
an immediate contact person and phone number to be reached
in the event additional information is needed. Failure
to have current beneficiary information on file could delay
payment of these funds, should they be needed. In
the event that current beneficiary information is not on
file, the benefit will be paid in the following order:
- Surviving spouse.
- Surviving parents.
- Surviving next of kin
(brother or sister)
- The person handling
the funeral arrangement.
It is the intent of the
association to provide immediate funds for the family to
use for funeral arrangements or other incidentals they may
need.

starts January 1st, 2008 but registrations are being accepted
now. Be sure to register your department right away.
